Job Description

Become a part of our caring community as a Home Health Speech Therapist, where you’ll play a vital role in helping patients regain strength, communication, and independence in the comfort of their homes. By delivering personalized, goal‑driven therapy, you help individuals overcome functional limitations, participate more fully in daily activities, and improve their overall quality of life.

As a Home Health Speech Language Pathologist, you will: evaluate, direct, and provide speech/language pathology services to patients in the home or facility; participate in the development and periodic review of individualized Plans of Treatment and Plans of Care; use your professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing, working to prevent, identify, evaluate, and minimize the effects of these conditions; administer and interpret diagnostic tests and therapeutic interventions, including audiologic screening; observe, record, and report changes in the patient’s condition and response to treatment to the supervisor and/or physician; instruct patients in the use of alternative communication systems when appropriate; counsel and educate patients, families, and other caregivers; maintain and submit clinical documentation as required, including evaluation reports, visit notes, care plans, care‑coordination activities, and progress summaries; participate in interdisciplinary care‑coordination and discharge planning; and uphold the highest standards of professional conduct, including strict protection of confidential information.
