

Home Health Speech Language Pathologist – PRN

Become a part of our caring community as a Home Health Speech Language Pathologist with CenterWell Home Health, reporting to the Clinical Manager. In this role you will help patients regain strength, mobility, independence, and confidence from the comfort of their homes by delivering personalized, rehabilitation-focused care.
As a Home Health Speech Language Pathologist, you will:
Evaluate, direct, and provide speech/language pathology services to patients in the home or facility.
Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
Use professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize the effects of such disorders.
Administer and interpret diagnostic tests and apply therapeutic treatments, including audiologic screening.
Observe, record, and report changes in the patient’s condition and response to treatment to the supervisor and/or physician.
Provide instruction and training to patients in use of alternative communication systems when appropriate.
Counsel and instruct patients, families, and healthcare staff.
Maintain and submit documentation as required, including written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports.
Participate in care coordination activities and discharge planning.
Maintain the highest standards of professional conduct regarding confidential information and share information only when clearly in the best interests of the patient.
Required Qualifications:
Minimum six months experience as a speech therapist / speech language pathologist.
Current and unrestricted state license as a Speech Language Pathologist.
Current CPR certification.
Valid driver’s license, auto insurance, and reliable transportation.
Preferred Qualifications:
Prior home health experience.
This role is patient-facing and part of Humana’s Tuberculosis (TB) screening program. It is also part of Humana’s driver safety program and requires maintenance of personal vehicle liability insurance at or above state minimums.
