Job Description

By delivering personalized care focused on rehabilitation and functional improvement, you empower individuals to overcome physical limitations, perform everyday activities with confidence, and enjoy a better quality of life.

We have both Per Diem and Full‑time opportunities, with patients located throughout Will, Ford, and Kankakee Counties and a territory‑based caseload for full‑time therapists. As a Home Health Speech Therapist, you will evaluate, direct, and provide speech/language pathology services in the home or facility; participate in development and periodic review of the Plan of Treatment and Plan of Care; use professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing; administer and interpret diagnostic tests and therapeutic treatments including audiologic screening; observe, record, and report changes in patient condition and response to treatment; instruct patients in alternative communication systems when appropriate; counsel and instruct patients, families, and healthcare staff; maintain and submit required documentation (evaluations, visit notes, summaries, care plans, care coordination activities, progress reports); participate in care coordination and discharge planning; and maintain the highest standards of professional conduct and confidentiality. You will also attend/participate in internal staff development programs and obtain continuing education as required.
