Become a part of our caring community as a Home Health Speech Language Pathologist with CenterWell Home Health, reporting to the Clinical Manager. In this role, you will help patients regain strength, mobility, independence, and confidence from the comfort of their homes by delivering personalized care focused on rehabilitation and functional improvement.
As a Home Health Speech Language Pathologist, you will:
Evaluate, direct, and provide speech/language pathology services to patients in the home or facility.
Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
Use professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize the effects of such disorders.
Administer and interpret diagnostic tests and therapeutic treatments, including audiologic screening.
Observe, record, and report changes in the patient’s condition and response to treatment to the supervisor and/or physician.
Provide instruction and training to patients in the use of alternative communication systems when appropriate.
Counsel and instruct patients, families, and healthcare staff.
Maintain and submit documentation as required, including written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports.
Participate in care coordination activities and discharge planning.
Maintain the highest standards of professional conduct regarding confidential information and share information only when clearly in the best interests of the patient.
Required Qualifications:
Minimum six months experience as a speech therapist / speech language pathologist.
Current and unrestricted state license as a Speech Language Pathologist.
Current CPR certification.
Valid driver’s license, auto insurance, and reliable transportation.

This role is patient-facing and part of Humana’s Tuberculosis (TB) screening program; selected candidates must complete TB screening. The role is also part of Humana’s driver safety program and requires a valid state driver’s license and maintenance of personal vehicle liability insurance at or above state minimums, or $25,000 bodily injury per person / $25,000 bodily injury per event / $10,000 property damage, whichever is higher.
