Job Description

Become a part of our caring community as a therapist at CenterWell Home Health, reporting to the Clinical Manager. You will help patients regain strength, mobility, and independence from the comfort of their homes by delivering personalized care focused on rehabilitation and functional improvement.

As a Home Health Speech Language Pathologist, you will: evaluate, direct, and provide speech/language pathology services to patients in the home or facility; participate in the development and periodic review of the Plan of Treatment and Plan of Care; use professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize the effects of such disorders and conditions; administer and interpret diagnostic tests and therapeutic treatments, including audiologic screening; observe, record, and report changes in the patient’s condition and response to treatment to the supervisor and/or physician; provide instruction and training to patients in use of alternative communication systems when appropriate; provide counsel and instruction to patients, families, and healthcare staff; maintain and submit documentation as required, including evaluation reports, visit notes, summaries, care plans, care coordination activities, and progress reports; and participate in care coordination activities and discharge planning while maintaining the highest standards of professional conduct and confidentiality.

This position offers flexible part‑time scheduling at 24 hours per week with benefits available or per‑diem options. The role is patient‑facing and part of Humana’s TB screening and driver safety programs, requiring TB screening, a valid state driver’s license, and personal vehicle liability insurance that meets state minimums or $25,000 bodily injury per person / $25,000 per event / $10,000 property damage, whichever is higher.
