In this home health role, you will play a vital part in helping patients regain strength, mobility, independence, and confidence from the comfort of their homes by delivering personalized care focused on rehabilitation and functional improvement, in line with Humana’s Caring, Curious, and Committed values.

As a Home Health Speech Therapist, you will evaluate, direct, and provide speech/language pathology services to patients in the home or facility; participate in the development and periodic review of the Plan of Treatment and Plan of Care; use professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize the effects of such disorders and conditions; administer and interpret diagnostic tests and therapeutic treatments, including audiologic screening; observe, record, and report changes in the patient’s condition and response to treatment to the supervisor and/or physician; provide instruction and training to patients in the use of alternative communication systems when appropriate; counsel and instruct patients, families, and healthcare staff; maintain and submit documentation as required, including written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress notes; participate in care coordination activities and discharge planning; and maintain the highest standards of professional conduct and confidentiality.
