Job Description

As a Home Health Speech Language Pathologist, you will:

  • Evaluate, direct, and provide speech/language pathology services to patients in the home or facility.

  • Participate in the development and periodic review of the Plan of Treatment and Plan of Care.

  • Use professional skills and judgment in assessing and treating disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize the effects of these conditions.

  • Administer and interpret diagnostic tests and therapeutic treatments, including audiologic screening.

  • Observe, record, and report changes in patient condition and response to treatment to the supervisor and/or physician.

  • Provide instruction and training to patients in use of alternative communication systems when appropriate.

  • Provide counsel and instruction to patients, families, and healthcare staff.

  • Maintain and submit required documentation, including written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports.

  • Participate in care coordination activities and discharge planning.

  • Maintain the highest standards of professional conduct concerning confidential information, sharing it only when the recipient’s right to access is clearly established and when doing so is in the patient’s best interest.

This role is part of Humana’s TB screening and driver safety programs and requires TB screening, a valid driver’s license, personal vehicle liability insurance at or above state minimum limits (or $25,000 bodily injury per person / $25,000 per event / $10,000 property damage, whichever is higher), and reliable transportation.
