Job Description

Become a part of our caring community as a Per Diem Home Health Speech Language Pathologist serving patients in the Iowa City area, with flexible scheduling based on your availability. As a Per Diem therapist at CenterWell Home Health, you play a vital role in helping patients regain strength, mobility, independence, and communication—all from the comfort of their homes—by delivering personalized care focused on rehabilitation and functional improvement.

As a Home Health Speech Language Pathologist, you will: evaluate, direct, and provide speech/language pathology services to patients in the home or facility; participate in the development and periodic review of the Plan of Treatment and Plan of Care; use professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize the effects of these conditions; administer and interpret diagnostic tests and therapeutic treatments, including audiologic screening; observe, record, and report changes in patient condition and response to treatment to the supervisor and/or physician; provide instruction and training to patients in use of alternative communication systems when appropriate; provide counsel and instruction to patients, families, and healthcare staff; maintain and submit required documentation, including timely written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress notes; participate in care coordination activities and discharge planning; and maintain the highest standards of professional conduct regarding confidential information, sharing it only when the recipient’s right to access is clearly established and when doing so is in the patient’s best interest.

You will attend, participate in, and/or conduct internal staff development programs and obtain continuing education as required by company policy and regulation.
