Become a part of our caring community as a Home Health Speech Language Pathologist with CenterWell Home Health, reporting to the Clinical Manager. In this home health therapy role, you’ll help patients regain strength, mobility, independence, and confidence from the comfort of their homes by delivering personalized care focused on rehabilitation and functional improvement.
As a Home Health Speech Language Pathologist, you will: evaluate, direct, and provide speech/language pathology services to patients in the home or facility; participate in the development and periodic review of the Plan of Treatment and Plan of Care; use professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing in order to prevent, identify, evaluate, and minimize the effects of these conditions; administer and interpret diagnostic tests and therapeutic treatments, including audiologic screening; observe, record, and report changes in the patient’s condition and response to treatment to the supervisor and/or physician; provide instruction and training to patients in the use of alternative communication systems when appropriate; and provide counsel and instruction to patients, families, and healthcare staff.
You will maintain and submit documentation as required by the company or facility, including written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress notes; participate in care coordination activities and discharge planning; and maintain the highest standards of professional conduct regarding confidential information, sharing it only when the recipient’s right to access is clearly established and when doing so is in the best interests of the patient. This patient-facing role is part of Humana’s TB screening and driver safety programs and requires TB screening, a valid driver’s license, auto insurance, and reliable transportation.
