In this home health therapy role, you help patients regain strength, independence, communication, and confidence from the comfort of their homes by delivering personalized, rehabilitation‑focused care.


As a Home Health Speech Therapist, you will evaluate, direct, and provide speech/language pathology services to adult patients in the home or facility; participate in the development and periodic review of the Plan of Treatment and Plan of Care; use professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize the effects of these conditions; administer and interpret diagnostic tests and therapeutic treatments, including audiologic screening; observe, record, and report changes in patient condition and response to treatment to the supervisor and/or physician; provide instruction in alternative communication systems when appropriate; and counsel and educate patients, families, and healthcare staff.careers.


You will maintain and submit documentation as required, including evaluation reports, visit notes, summaries, care plans, care coordination activities, and progress notes; participate in care coordination and discharge planning; and uphold the highest standards of professional conduct and confidentiality, sharing information only when the recipient’s right to access is clearly established and when doing so is in the patient’s best interest. This patient‑facing role is part of Humana’s TB screening and driver safety programs and requires TB screening, a valid driver’s license, auto insurance, and reliable transportation.
