Job Description

Become a part of our caring community as a Home Health Speech Language Pathologist, reporting to the Clinical Manager and providing personalized, rehabilitation‑focused care in patients’ homes. As a Home Health Speech Language Pathologist, you will evaluate, direct, and provide speech/language pathology services in the home or facility; participate in the development and periodic review of the Plan of Treatment and Plan of Care; and use professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize the effects of such disorders and conditions. You will administer and interpret diagnostic tests and therapeutic treatments (including audiologic screening); observe, record, and report changes in patient condition and response to treatment; and provide instruction and training in the use of alternative communication systems when appropriate. The role includes counseling and instruction for patients, families, and healthcare staff, maintaining and submitting documentation (evaluations, visit notes, summaries, care plans, care coordination activities, and progress reports) in accordance with company policy, and active participation in care coordination and discharge planning. You are expected to maintain the highest standards of professional conduct and protect confidential information, sharing it only when the recipient’s right to access is clearly established and doing so is in the patient’s best interest.
