  • Effective communication and counseling skills with patients, families, and healthcare staff.
  • Commitment to patient confidentiality and high professional standards.
  • Preferred: prior home health experience.

Job Description

Become a part of our caring community as a Home Health Speech Language Pathologist with CenterWell Home Health, a Humana company. Reporting to the Clinical Manager, you will play a vital role in helping patients regain strength, mobility, independence, and confidence from the comfort of their homes by delivering personalized care focused on rehabilitation and functional improvement.

As a Home Health Speech Language Pathologist, you will evaluate, direct, and provide speech/language pathology services to patients in the home or facility; participate in developing and reviewing the Plan of Treatment and Plan of Care; use professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing to prevent, identify, evaluate, and minimize the effects of these conditions; administer and interpret diagnostic tests and therapeutic treatments, including audiologic screening; observe, record, and report changes in patient condition and response to treatment to the supervisor and/or physician; provide instruction in alternative communication systems when appropriate; and counsel and instruct patients, families, and healthcare staff.

You will maintain and submit documentation as required by the company and/or facility, including written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress notes. You will participate in care coordination and discharge planning and maintain the highest standards of professional conduct and patient confidentiality, sharing information only when the recipient’s right to access is clearly established and doing so in the best interest of the patient. This patient-facing role is part of Humana’s TB screening and driver safety programs and requires TB screening, a valid driver’s license, auto insurance, and reliable transportation.
